Volunteer Recognition Breakfast
At
9 a.m. on April 24, the Kokomo community will gather at Crossroads
Community Church to celebrate its outstanding volunteers. These
volunteers LIVE UNITED by sharing their time and talent to better our
community.
Below are the nomination forms and guidelines for the 2009 Volunteer
Awards. The awards will be presented during breakfast, which is
sponsored by the United Way of Howard County, Please take a moment of
your time to nominate a friend, co-worker, retriee, student, group or
senior citizen who has made an impact in our community by volunteering.
You are encouraged to submit more than one nomination.
Please completely fill out the nomination form and add all supporting
information as requested, and mail or fax the information packet to 210
W. Walnut Attn: Cheryl Graham. All nominations are due by April 3,
2009.
Nomination Eligibility Information:
* Volunteer services must be performed in the
Howard County area.
* Work must be completed at a non-profit
organization/agency.
* Nominee’s volunteer must serve the community at
large. Volunteers solely engaged in
member only projects are ineligible. Such as religious, fraternal, or
professional groups providing services to their members only.
* More than one agency may nominate the same individual/group.
* Individual Youth Award-nominees must be18 years
or younger at time of nomination.
* Youth groups are eligible for the Group or
Education awards only.
* Nominees are eligible in one category only.
* Group Award–2 or more individuals.
* United Way Staff, co-sponsors, judges,
Chairperson and their family members cannot participate as a nominator or
nominee.
* Nominees
cannot be selected as Winner in the same category in two consecutive years.
